How this usually happens
Almost nobody in this category set out to misuse anything
Ambien is prescribed for short-term insomnia — typically a few weeks. What happens in practice is that the prescription gets renewed, sleep never returns to normal without it, and two years pass.
Tolerance builds. The original dose stops working, and stopping produces sleep so much worse than the original problem that continuing looks like the only option. That is not misuse. It is a predictable pharmacological outcome of nightly use continued past the intended window.
The marketing did not help. Z-drugs were introduced as a safer class than benzodiazepines, and that framing has stuck long past the evidence. They bind the same GABA-A receptor complex, produce comparable dependence, and carry a comparable withdrawal risk profile.

The symptom that decides it
Rebound insomnia is why people stay on it for years
Ask anyone who has tried to stop Ambien what happened and the answer is almost always the same: the first few nights without it were worse than any insomnia they had before starting.
That is rebound, and it is real. Sustained Z-drug use suppresses normal sleep architecture; removing the drug leaves the system to reassemble it, which takes weeks rather than days. In the meantime sleep is genuinely worse than baseline.
People experience that, reasonably conclude they cannot sleep without it, and refill. The loop closes.
What breaks it is a gradual taper plus non-habit-forming sleep support through the rebound window, and knowing in advance that the window exists and ends. That last part matters more than it sounds.

Do not stop abruptly, and mention the sleepwalking
Abrupt cessation after sustained or high-dose Ambien use can cause seizures, as with benzodiazepines. That includes running out of a prescription.
Complex sleep behaviors — sleepwalking, sleep-driving, eating with no memory of it — are documented with Ambien and carry an FDA boxed warning. People are often embarrassed to raise it. It is clinically relevant and worth saying at assessment.
